During open enrollment, you can make changes to your health plan or switch to a
different plan. Please select your state below.

Important: You can only use this form to change your plan if it was purchased
directly through Kaiser Permanente. To make a change or switch to a new plan
through the health benefit exchange, please apply directly with the exchange.

If you live in Benton, Columbia, Franklin, Island, King, Kitsap, Kittitas,
Lewis, Mason, Pierce, San Juan, Skagit, Skamania, Snohomish, Spokane, Thurston,
Wahkiakum, Walla Walla, Whatcom, Whitman, or Yakima county

 * Download and complete an Account Change Form and mail or fax it back to us
 * Call our Member Service Contact Center at 1-888-901-4636 (TTY 711)

 * Add a spouse, domestic partner, or dependent to your current plan.

FINANCIAL HELP

Financial help won't apply if you sign up for a plan directly through Kaiser
Permanente. To get financial help, you'll need to sign up through the health
benefit exchange.


WHEN CAN I CHANGE PLANS?

You can only change plans during open enrollment, which starts November 1, 2023.
For coverage starting January 1, 2024, you'll need to sign up for a new plan by
December 15, 2023. Outside of open enrollment, you can only change plans if you
have a qualifying life event.
