INFORMATION ABOUT THE ONTARIO HEALTH SELF-TEST WEBSITE

To use this website you will need to have picked up a COVID-19 self-test kit
from a health care provider (such as a pharmacy, hospital or other health care
facility, referred to as “your ordering clinician”) and access to the applicable
Ontario photo health card for the individual seeking a COVID-19 self-test.

This Ontario Health Self-test website is a web-based tool (this "Website") that
will enable you to register the COVID-19 self-test kit that you picked up from
your ordering clinician for processing by a provincially accredited medical
laboratory. To use this Website your ordering clinician should have already
provided you with a COVID-19 self-test kit that will allow the collection of a
specimen for a COVID-19 test (in accordance with the directions with that kit).

The COVID-19 self-test kit includes a code that identifies the kit, as well as
your ordering clinician. This Website will require you to scan that code (or
type it in if you are unable to scan it) and enter certain identifying
information about the individual whose specimen is being tested, such as their
name, health card number, contact information (email address or mobile number),
etc., all of which is personal health information as defined by the Personal
Health Information Protection Act, 2004 (“PHIPA“). Please note that you are
required to provide the above-specified information, including contact
information (email address or mobile number), in order for you to register with
this Website; if you do not provide this required information then you are not
able to register with this Website. Once you have registered through this
Website, you are to return the collected specimen sample to your ordering
clinician, which will submit the specimen sample for pick-up and testing by a
provincially accredited medical laboratory.

This Website may be used to register a COVID-19 self-test kit on your own behalf
and on behalf of another individual that you have determined you are authorized
to act on behalf of in accordance PHIPA (for example: you are the parent of
child entitled to consent on behalf of a child who is under the age of 16).

If you have any questions about who accesses or uses the information submitted
with an order for a COVID-19 self-test, what is involved in processing the
specimen, or who will have access to the results, please contact your ordering
clinician (not Ontario Health).

This Website is not intended to provide medical advice. If you have medical
questions, contact your doctor or health care provider.

This Website does not provide you with access to the results of any COVID-19
test or any other medical information. Any results from this service will be
available through the Ontario Laboratories Information System (“OLIS”), a system
of the Ministry of Health (Ontario), through the site listed below.

ACKNOWLEDGEMENT

By using this Website to place an order for a COVID-19 self-test, you consent to
your ordering clinician collecting the personal health information that you
submit through this Website so that your ordering clinician can ensure that it
can correctly identify the individual to whom the COVID-19 self-test applies,
and then submit that personal health information and specimen sample for
processing by a provincially accredited medical laboratory. You also consent to
notifications in connection with the applicable order being sent to the email
address or mobile number you provide, and accept that messaging by email or
texting (short message service or SMS) contain elements of risk, such as theft
of mobile device, message loss, account compromise or interception by
unauthorised persons. This Website will only use your provided email address or
mobile phone number for the purposes of providing notifications in connection
with the applicable order. Standard messaging rates will apply. In addition, you
understand and agree that Ontario Health does not warrant or guarantee that you
will receive any notification concerning results. Once testing is completed and
submitted to OLIS, your results will be available through the COVID-19 test
results viewer at: https://covid19results.ehealthontario.ca:4443/agree.

By using this Website to submit personal health information and create an order
on behalf of another individual, you are attesting that you have determined you
are authorized to act on behalf of this other individual in accordance with
PHIPA, and you consent as set out in the preceding paragraph.

SECURITY

You are responsible for the security of the device, browser, and network that
you are using to access this Website or to receive any messaging by email or
SMS.

AUDIT

By using this Website, you acknowledge that your access to and use of this
Website may be securely logged and audited to respond to an inquiry from you,
and to investigate any privacy breach incidents.
