WE PUT PEOPLE AT THE CENTRE OF EVERYTHING

Our Fusion Model is based on Person-Centred Active Support (PCAS) – a way of
supporting people with learning disabilities so they are engaged in meaningful
activity and relationships as active participants. As a result, they exercise
more control over their lives and experience greater levels of inclusion,
independence, and choice. PCAS is incorporated into induction for new starters
to ensure that everyone at Hft understands this way of working right from the
beginning of their career with us. PCAS is what we do…

   With supported living, we support people to live however and wherever they
   choose. We can help with finding the right accommodation and with ongoing

   We provide domiciliary care to people who require support with activities
   they would find difficult on their own, to help them live with more

   Our Day Opportunities centres allow the people we support to take part in

   process. Our tips are intended to make the process of completing the form as
   straightforward as possible. Find out more.
 * Is there a difference between a learning disability and a learning
   difficulty?
 * In the UK we use the term ‘learning disability’, often interchangeably with
   ‘learning difficulty’. While it's not always helpful to label, at Hft, when
   we talk about a learning difficulty, we’re usually talking about a condition
