IMPORTANT UPDATE




Effective April 1, 2022, Protected Personal Identifying Information must not be
included in any public document or attachment filed with the court. This
includes any person’s

 * Date of birth
 * Social security number or national identification number
 * Driver’s license number or state-issued personal identification card number
 * Passport number
 * Financial account numbers

If any of the public documents you plan to submit have any of this information,
it must be provided on a separate form provided by the State Court
Administrative Office (MC97, MC97a, or MC97b).To access these forms, and for
more information on protecting personal identifying information, please click
here.
