Unlimited primary care1

Virtual visits conducted by telephone or video chat with a doctor are not an
insurance product, health care provider or a health plan. Unless otherwise
required, benefits are available only when services are delivered through a
Designated Virtual Network Provider. Data rates may apply. Virtual visits are
not intended to address emergency or life-threatening medical conditions and
should not be used in those circumstances. Services may not be available at all
times, or in all locations, or for all members, and visit limits may apply.

Unlimited virtual urgent care1

Tier 1 prescriptions at Walgreens2

Pay a $3 copay on preferred generic prescription drugs. Not available on all

We offer a variety of Individual & Family ACA Marketplace plans that we classify
into 4 categories. Within these categories, “metal” levels tell you how costs
are divided between you and UnitedHealthcare. Learn about our plan options that
fit your budget and your needs.

ESSENTIAL HIGH-DEDUCTIBLE PLANS WITH BASIC COVERAGE FOR SERIOUS ILLNESS OR
INJURY. PAY A LOW OR $04 MONTHLY PREMIUM, BUT MORE WHEN YOU GET CARE.

WHAT TO KNOW

This is our lowest-cost plan. It’s a good fit if you want a low-cost plan with
benefits for worst-case medical scenarios, like serious illness or injury. Your
monthly premium could be as low as $0,4 but you’ll pay more when you get care,
compared to our other plans.4 Essential plans are available at the Bronze metal
level only.

To qualify for a $0 monthly premium, you must meet household income requirements
for Advanced Premium Tax Credits.

You rarely see your doctor. You want a low-cost way to protect yourself from
worst-case medical scenarios, like serious illness or injury.

Built with flexibility and convenience on the go, this plan offers unlimited
24/7 $05 virtual care through the Galileo mobile app. This plan is a good fit
if you’d rather talk, text or have a video visit with a doctor instead of
heading to the clinic. Your 24/7 access to virtual care lets you interact with
your doctor on your schedule, when it’s most convenient for you. The Galileo
mobile app will be your first stop for all your care needs, from everyday to
urgent — including primary and specialty care. Virtual First plans are available
at the Bronze and Silver metal levels (Gold Virtual First available in Maryland
only).

Virtual First health plans offer access to virtual primary and specialist care
coordinated through Galileo, a virtual primary care provider. Members of this
plan must have a smartphone or tablet, and data rates may apply. Virtual First
health plans are not available in all states. Members have no cost sharing for
care provided through the Galileo app. For in-person care, cost sharing (which
may include deductibles, copays and coinsurance) may apply. Certain
prescriptions may not be available, and other restrictions may apply. Virtual

WHAT EXACTLY ARE ACA MARKETPLACE HEALTH INSURANCE PLANS?

ACA Marketplace plans are health care plans that people can buy on their own,
rather than through an employer or another government-run program, like Medicare
or Medicaid. You may also hear names like ObamaCare and Individual & Family
Plans. The health care Marketplace (also called the Exchange) is where ACA
health care plans are sold.

HOW CAN I SAVE MONEY ON THE HEALTH CARE MARKETPLACE?

Under the American Rescue Plan Act of 2021 (ARPA) and Inflation Reduction Act of
2022, you may be able to enroll in ACA Marketplace coverage with lower premiums
and see if you qualify for financial subsidies. These subsidies could be tax
credits or cost-sharing reductions to help pay for your health care costs.

WHEN CAN I ENROLL?

2023 Open Enrollment starts Nov. 1, 2023, and ends Jan. 15, 2024, in most
states.

3. Members age 18 and over can earn a $100 gift card for select retail
participants upon completion of four activities: 1) Member sees their
primary care provider for a wellness visit; 2) Member signs up for all types
of text messages from UnitedHealthcare; 3) Member opts in to all types of
paperless communications; and 4) The subscriber of the plan signs up for
auto-pay of premiums on behalf of all members enrolled on their plan. Limit
one gift card per eligible member. Eligible members who are unable to
participate in an available program may be permitted to earn the same reward

5. Virtual First health plans offer access to virtual primary and specialist
care coordinated through Galileo, a virtual primary care provider. Members
of this plan must have a smartphone or tablet, and data rates may apply.
Virtual First health plans are not available in all states. Members have no
cost sharing for care provided through the Galileo app. For in-person care,
cost sharing (which may include deductibles, copays and coinsurance) may
apply. Certain prescriptions may not be available, and other restrictions
may apply. Virtual care is not appropriate in all circumstances, including
for emergencies or life-threatening medical conditions, and should not be
used in those circumstances
6. Adult Dental and Vision: $0 cost-share for in-network periodic oral
evaluation 2 times per consecutive 12 months, coverage is limited to a
$1,000 benefit maximum. Routine vision exam and eyewear are limited to once
every 12 months. If you opt to receive vision care services or vision care
materials that are not covered benefits under this plan, a participating
vision care provider may charge you their normal fee for such services or
materials. Coverage may not be available in all plans or in all states. For
