I love being the school nurse at Bellerive. I believe students that feel
safe and healthy at school are the foundation of excellence in the classroom.
By helping students feel safe, comfortable, and healthy it will help them
focus, stay on task, and succeed in the classroom. The staff here at
Bellerive are exceptional in not only teaching your children academics, but
also how to be caring, respectful, and responsible individuals.  The Seven
Habits help to guide the children in their decision-making daily, and we hope
   in many years to come.

It is my goal as the School Nurse to focus not only on each child’s physical
health and well-being, but also their emotional and mental health. There are
times when emotional or mental challenges in life can manifest themselves as
physical signs and symptoms. Please do not hesitate to let myself, Counselor,
or other staff members know if your child may be experiencing some challenges
that may present themselves while here at school. All staff members are here
for the betterment of each child.  We will find appropriate resources to
enable your child to be the most successful possible. Community Resources are
   also available online.

   Illness

When to keep your child home from school:

Fever  - a temperature of 100.0 F or greater.  Please do not send your child
to school until he/she has been fever-free for 24 hours, without the use of
   an Analgesic.

Vomiting or diarrhea – should have ceased for 24 hours before returning to
school. Some children may need longer than 24 hours to recuperate from a
   stomach virus.

Rash – Please do not send your child to school until your Doctor has said it
is safe to do so. A child returning to school with a rash will require a
Physician’s Note stating it is not a communicable disease and is safe to be
around the other children at school.

Head Lice – Beginning the Fall of 2015, Parkway has a No-Exclusion Policy for
Head Lice. A child who is found to be infected with live head lice will be
referred to his/her parent/guardian for treatment. Once the child has been
treated, he/she may be re-admitted to school upon inspection by the School
   Nurse.

Sore Throat  - If your child has a sore throat, and does not have a fever
(without the aid of an Analgesic), or any other symptoms, it is ok to send
   him/her to school.

Coughing – If your child is coughing uncontrollably, it is best to keep
him/her home and let them rest before returning to school.

   Medications

Over-The- Counter Medications (OTC) - Requires a Parent Form for OTC up to 5
day consecutively. After the 5 days a parent note as well as a Physician Form
is needed before the School Nurse is able to administer the medication at
school.  The School Nurse has a Standing Order from the District Physician to
administer Caladryl and Antibiotic Ointment; all other OTC require a
Physician’s written order and Parent Permission.  Tylenol can be administered
   for a fever of/greater than 101.0 F only.

*The Parent Form and the Physician Form/Physician Form should state the name
of the child, name of the Medication, dosage, schedule of administration, and
   indications for giving the Medication.

Prescription Medication – a Parent Form and Physician form must be completed.
Both the Parent Form, Physician form,  and Prescription bottle will
correspond with one another for the School Nurse to be able to administer the
   medication at school.

   Immunizations

Vaccinations immunize an individual and are meant to protect a person from a
serious illness or disease. The U.S. Centers for Disease Control's Advisory
Committee on Immunization Practices (CDC) publishes a new schedule annually
showing which vaccines are recommended and when to get them.  Each state
determines the requirements for their population.  Immunizations not only
protect the individual, but also those in the community/school – students and
   staff.

   * Each student must be compliant with their Immunizations in order to start
   their first day of school.

*Please do not hesitate to contact the School Nurse, if you have any
   questions regarding immunizations/vaccines.

   Food Allergies

If your child has a diagnosed Food Allergy, please let the School Nurse
know.  If your child has an intolerance or sensitivity to certain foods or
ingredients, please also let the School Nurse know about this.  Please
provide an Epi Pen as prescribed by your Physician, as well as a completed
Food Allergy Action Plan (Parent Form must also accompany these two items).
Please let the School Nurse know if it is necessary for your child to sit at
the Food Allergy Aware Table at lunchtime in the Café.

*If you have any questions regarding your child and his/her Food Allergies at
school, please do not hesitate to call the School Nurse.

   Asthma

If your child has diagnosed Asthma, please let the School Nurse know.  Please
provide an Asthma Action Plan (provided by Physician), inhaler, and Parent
Form if it is necessary for your child to keep an inhaler in the Nurse’s
   Office at school.

*If you have any questions regarding your child and Asthma at school, please
do not hesitate to call the School Nurse.

   Injuries

When to alert your School Nurse:

   1)  If your child becomes injured outside of school and you need to seek
   Medical attention

   2)  If your child will need extra accommodations while at school (ie:
extended time – crutches, walking boot, etc); this would include not only
physically noticeable injuries, but brain injuries as well

   3)  If your child will not be able to participate in PE (a
Physician’s/Medical Provider’s Note will be necessary beyond one or two
classes; a Parent Note is accepted for the first one or two classes missed)

*Please provide a Physician’s/Medical Provider’s Note to your School Nurse
upon the return of your child, especially if he/she will need to have
   restrictions.
