HEALTH SCREENINGS

The Montana Health Centers can perform your annual health screening. These will
be performed as directed by your Health Plan Live Life Well Incentive program.
For State of Montana plan members: A State-sponsored health screening includes
measurements as described below at a Montana Health Center or off-site event. A
State-sponsored health screening is a benefit for ALL plan members (including
Medicare-eligible retirees) once per incentive period. All personal information
is protected by HIPAA.

Appointments are required.  Attendees must follow the guidelines listed below
under "What to Expect," due to COVID-19 restrictions.  Please schedule now to
ensure you get the date and time you prefer and to ensure staff has sufficient
time to prepare for your appointment and sanitize before the next member.

What to Expect

 * All patients coming to a health screening event will be asked COVID screening
   questions and have their temperature taken. If you have contact with anyone
   with a suspected or confirmed case of COVID-19 and/or are running a
   temperature of 100 degrees Fahrenheit or greater, PLEASE do not attend the
   health screening. Contact Premise Health so your appointment can be
   rescheduled. Answer the COVID screening questions honestly. They are in place
   to reduce exposure and allow us to continue the health screening events.
 * Patients must bring their own face mask and wear it into the screening
   location. If you cannot gain access to a face mask, please let the health
   center know prior to arriving for your appointment.
 * Only patients will be allowed to enter the screening site - family members or
   other guests will not be permitted if not necessary.
 * Waiting areas are limited to 2-3 patients at one time. If more than 3
   patients are waiting to be seen, excess patients will be requested to wait in
   another location until their turn. Once inside the off-site screening event
   maintain at least 6 feet of distance from others if possible.
 * Screening staff will draw your blood, do a height and weight measurement, and
   take your blood pressure.
 * You may request that staff measure your waist circumference. This measurement
   is optional. Visit the National Heart, Lung, and Blood Institute website for
   more information on what waist circumference and BMI tell you about your
   health.
 * Health screenings are provided in a private, comfortable environment. If you
   have any concerns, please communicate them to the health screening staff when
   you check in.
 * Health screenings take about 20 minutes.

Lab Tests

A Lipid Panel, Comprehensive Metabolic Panel (CMP), and Complete Blood Count
(CBC) are included in your health screening.

The following tests may be included with your health screening IF you bring a
prescription/doctors’ orders with you to your screening.

 * HgbA1C
 * PSA
 * TSH (thyroid test)

At the health centers ONLY - additional lab (blood) tests can be added if you
bring a prescription/doctor's orders with you to your health screening. 

Health Screening DO'S

 * Bring a face mask.
 * Do drink plenty of water in the 24 hours prior to your screening.
 * Do take prescription medications as directed by your doctor.
 * Do wear clothing with loose fitting sleeves to the screening.
 * If you are diabetic or pregnant: Schedule your appointment earlier in the
   morning so fasting time does not extend late into the morning. If there is
   any reason why it may not be appropriate to fast, please contact your Health
   Plan for assistance.
 * It is okay to brush your teeth prior to the screening.

Health Screening DON'TS

 * Don't attend your appointment if you are sick.  Call Premise Health at (855)
   200-6822 to reschedule your appointment.
 * Don’t eat or drink food or beverages other than water and black coffee 12
   hours prior to your screening.
 * Don’t exercise strenuously 12 hours prior to your screening.
 * If you cannot attend your scheduled appointment for any reason, please be
   courteous and cancel your appointment as soon as you know you won't be able
   to make it. 

Eligibility 

Employees, spouses, retirees and dependents on the State of Montana health plan
age 18 or older are eligible for one health screening per State Plan year. This
program is voluntary.

Live Life Well Incentive

Part of earning the Live Life Well Incentive is completing a State-sponsored
health screening between November 1, 2021 and October 31, 2022. Click HERE to

WIN A $50 GIFT CARD!

We are encouraging all State Plan members to get their State-sponsored health
screening early in 2022.  Members who complete a health screening from November
1, 2021 through August 31, 2022 will be entered to win a $50 gift card after
they have completed their screening!  5 winners will be drawn at the end of each
month from February through August.  Winners will be notified using their
official contact information. 

Not only does this help you avoid the end of the year rush and possible issues,
but it also greatly assists the Montana Health Center in meeting your
State-sponsored health screening needs.  Many of us have a tendency to wait
until later in the Incentive year to complete our screening.  This creates a
strain on the resources available, frustration for health center staff and Plan
members, problems that don’t have time to be resolved, and fewer appointments
being available for flu shots, regular medical visits, and acute care visits. 
It is in all our best interests if we can get our screenings done earlier in the
year!

Once you have completed your health screening, you will be entered to win each
month starting in February through August. 

 * Example 1: If you complete your screening in December of 2021, your name will
   be entered for our monthly drawing February, March, April, May, June, July,
   and August.
 * Example 2: If you complete your screening in April 2022, your name will be
   entered for our monthly drawing April, May, June, July, and August.

Members are eligible to win one $50 gift card. 

Health screenings completed after August 31, 2022 (for any reason) are not
eligible for this prize drawing. 

Congratulations to the March $50 gift card winners!  Look for another drawing at
the end of April.
