Go to the UHC website and log in to your account (Click on this link if you haven't registered yet) 2. Once logged into myUHC.com, Under Providers and Facilities on your Dashboard you'll find your PCP 3. Under their name, Click on the link to CHANGE PCP 4. Click on Find a New PCP 5. Verify you have your correct ZIP Code entered 6. In the Search field, type the new doctors name to see if they're contracted, or 7. Click on All Primary Care Physicians, just below the Search field to find a new In-Network PCP 8. Choose your new PCP from the list of results that are listed based on proximity to the ZIP Code entered 9. Once you've chosen a new PCP, Click on SELECT PCP, located just under their name 10. Select the Location where you will visit this PCP 11. Click ASSIGN AS PCP and your PCP change request is submitted for processing. Note: If there's any error in processing this PCP change the system will prompt you to call Member Services or request that they contact you. Note: Depending on your plan, we will send you a new UHC/NHP ID card that shows the name of your new primary care provider. You can change your doctor as needed, up to once per month. If you need a more immediate change, please call the Customer Service number on your UHC/NHP ID card.
