What is 100Plus?

100Plus is the only AI-assisted remote patient monitoring (RPM) platform that
enables healthcare providers to remotely manage their patients’ chronic
conditions, and proactively supports practices with patient outreach, device
setup, ongoing support and education, ensuring patients stay compliant. The
result is dramatically higher patient adherence, improved health metrics, fewer
hospitalizations, and better patient outcomes.

As part of a remote patient monitoring program, providers are reimbursed fully
by Medicare, (and certain Medicaid and insurance providers) for setup, hardware,
service costs, and ongoing review of patient data. Eligible Medicare patients
receive devices at no cost3.

What is Remote Patient Monitoring (RPM)?

Remote patient monitoring (RPM) is a healthcare delivery method that uses
digital technologies to monitor and analyze a patient’s vitals and other health
data outside of a traditional clinical setting. RPM technology electronically
transmits health information between patients and physicians. This type of
monitoring is often used to manage and care for high-risk patients, such as
those with chronic and/or acute health conditions.


What are the most common medical specialties that offer remote patient
monitoring?

Specialties that most commonly utilize RPM include cardiology, pulmonology,
endocrinology, gastroenterology, bariatrics, internal medicine and primary care.

What are some of the benefits of using RPM?

RPM provides a wide range of benefits, including:

Is there a copay associated with RPM services?

Yes. For Medicare beneficiaries there is a 20% beneficiary copay. However, this
copay may be covered if a patient has supplemental coverage or Medicaid. Private
payers will vary and may or may not require a copay for RPM services.

How do I get 100Plus remote monitoring devices for my patients?

There are two ways you can enroll patients in your 100Plus RPM program: 

1) 100Plus white-glove end-to-end enrollment service can help identify eligible
patients for your practice, perform patient consent, enrollment and ongoing
support. 
2) If you have identified a patient with at least one chronic condition and
obtained their consent to participate in our RPM program, you can enroll the
patient via the 100Plus portal.

It’s a simple 3-step process:


 * Enter their basic info (name, address, etc.).
 * Have the patient sign the 100Plus User Agreement.
 * The 100Plus device(s) will be shipped to the patient; the device will be
   ready to use with cellular connectivity enabled.

The device will be associated with the medical provider who recommended it. Your
patient can simply open the package and start to use it. It’s required that you
obtain consent from your patients to provide remote patient monitoring services
and important that you discuss your recommendation and the program with your
patient so they understand its benefits and the importance of their continued
usage of the device once they receive it. These steps are simple — we will
review it during the remote training session. 

Where can I track my patients’ activity?

The 100Plus portal contains all the information you need to monitor your
patients’ activity. You can track patient enrollment, see if they have provided
all the necessary information to receive a device, including signing the User
Agreement, and track their device shipment. Once they start using their device,
you can also see all of their clinical data.

How do I get reimbursed?

Medicare reimbursement rules compensate medical providers for prescribing and
reviewing data produced by remote patient monitoring devices that capture,
store, and transmit patients’ physiological data and subsidize the device and
service costs.

Every month, you receive up to $182 per patient per month, or $2184 per patient
per year, for CPT 99454, 99457, and 99458 (2x). In addition, you receive a
one-time reimbursement for setup and patient education of up to $18.48 per
patient4.

Can RPM be billed in conjunction with chronic care management (CCM)?

Yes, a provider can bill for both CPT 99457 and CPT 99490 in the same month.
This is because CMS recognizes that the analysis involved in providing RPM
services is complementary to CCM and other care management services. However,
time spent delivering these services cannot be counted towards the required time
for RPM and CCM codes for a single month. Billing 99457 and 99490 together
requires a provider to deliver at least 40 minutes of services: 20 minutes of
RPM, 20 minutes of CCM.

‍CMS Guidelines: For patients with two or more chronic medical conditions
expected to persist for more than 12 months following diagnosis and place the
patient at an increased risk for hospitalization.

‍99487: Minimum of 60 minutes non-face-to-face consultation establishing or
monitoring a care plan
‍
‍99489: For every additional 30 minutes non-face-to-face consultation (with
99487)
‍
‍99490: Minimum 20 minutes non-face-to-face consultation monitoring the care
plan
