REPORT SUSPICIOUS ACTIVITY

Your smartphone has more in common with your computer than with your old mobile
phone. Criminals know you're literally carrying a small PC in your pocket and
will do all they can to access your personal information. Following a few simple
steps you can dramatically reduce your risk of having your money and identity
stolen. See the tabs below for suggestions. To protect the security of your U.S.
Bank account, we’ve expanded our authentication options to now include
fingerprint scan, one-time passcode, visual pattern and facial recognition.






IF YOUR CARD IS LOST OR STOLEN


 * Call 877-595-6256 immediately to report lost or stolen U.S. Bank credit and
   debit cards.

REPORT MISSING REPLACEMENT CARDS

 * When your card expires, we will send you a new one. If your mail is stolen,
   your new card may fall into the wrong hands. Call us if you don’t receive a
   replacement card before the expiration date listed on your current card.




SUSPICIOUS ACCOUNT ACTIVITY


 * Monitor your statements closely, and immediately report any transactions you
   don’t recognize to the U.S. Bank Fraud Liaison Center at 877-595-6256.
 * To report fraud or get other assistance related to your ReliaCard, please
   contact ReliaCard cardholder services at 855-282-6161.
 * Consider signing up for U.S. Bank’s free security alerts service to help you
   identify and stop potential fraud. Learn more about U.S. Bank security
   alerts.

REPORT SUSPICIOUS ACTIVITY ONLINE

 * Log in to online banking, then select Customer Service Center on the left
   menu.
 * Select report account issues.
 * Select the link that addresses your specific issue.




BEWARE OF SUSPICIOUS EMAILS


 * Criminals may send you “phishing” emails designed to trick you into
   downloading malware or revealing confidential banking information Learn the
   signs of a phishing email.


HOW TO REPORT SUSPICIOUS EMAILS

 * If you receive a suspicious email, immediately forward it to
   fraud_help@usbank.com. Your quick action helps U.S. Bank fight fraud.
 * This email box should only be used to forward fraudulent emails or text
   messages to U.S. Bank for investigation.




BE WARY OF SUSPICIOUS TEXT MESSAGES


 * Scam artists may send a text message to your mobile phone that tries to trick
   you into revealing confidential banking information. This practice is called
   “smishing” Learn how to recognize suspicious text messages.


HOW TO REPORT SUSPICIOUS TEXT MESSAGES

 * If you receive a suspicious text message, immediately forward it to us at:
   fraud_help@usbank.com. Reporting helps us fight fraud.
 * This email box should only be used to forward fraudulent text messages or
   emails to U.S. Bank for investigation.




SUSPICIOUS PHONE CALLS OR VOICE MESSAGES


 * People who call and ask for confidential banking information are most likely
   criminals.
 * These people may sound legitimate, but don’t believe them. U.S. Bank will
   never contact you to ask for personal information.
 * We will only request confidential information to verify your identity when
   you initiate contact with us.

HOW TO REPORT SUSPICIOUS PHONE CALLS OR VOICE MESSAGES

 * If you receive a suspicious phone call or voice message, immediately call us
   at 800-USBANKS (872-2657).




REPORT IDENTITY THEFT


 * Identity theft is the illegal use of someone else's personal information
   (such as a Social Security number or bank account number) in order to obtain
   money or credit.
 * If you think someone has stolen your identity, report it immediately to the
   U.S. Bank Fraud Liaison Center at 877-595-6256.
