Impression Kit rebate Offer

OFFICIAL TERMS This offer ("Offer") is only open to legal residents of the
United States (including D.C.), Canada, United Kingdom, Ireland, Australia, New
Zealand and Hong Kong. To obtain your SmileDirectClub rebate, awarded in the
form of a virtual pre-paid Visa card ("Card"), you must purchase a remote
impression kit ("Impression Kit") online from SmileDirectClub no later than
December 31st, 2022 and enter an eligible promo code where directed ("Eligible
Purchase"). The Card will be issued in the amount of the price paid for the
Impression Kit excluding any applicable taxes, shipping, and/or handling costs,
up to $49 USD, $65 CAD, £39, €49, $79 AUD, $69 NZD or $400 HKD. After completing
an Eligible Purchase and returning your completed Impression Kit, a digital
claim code ("Digital Claim Code") and redemption instructions will be emailed to
you within approximately ten (10) business days of receipt of your completed
Impression Kit and digital photos. This email will be sent to the email address
provided at the time of your Eligible Purchase. To receive the Card, you must
redeem the Digital Claim Code within four (4) months of the date it was emailed
to you, by following the redemption instructions provided. SmileDirectClub, LLC
("Sponsor") is not responsible for changes of email address or for any technical
or communication issues. If you do not receive your Digital Claim Code within
approximately fifteen (15) business days of the date you returned your
Impression Kit, please contact support@smiledirectclub.com for further
assistance. Digital Claim Code may only be used once. The Card is subject to the
terms and conditions imposed by the issuer, Peoples Trust Company, and must be
redeemed or used within seven (7) months of the date of issue. The Card is
non-transferable and may not be redeemed for cash. For complete terms and

person and per Eligible Purchase. Offer is valid only in the United States,

will not receive a Card. Sponsor is not responsible for (i) incorrect or
inaccurate capture of information for any reason; (ii) late, lost, damaged, or
garbled transmissions or transactions; (iii) Eligible Purchases that have been
made through illicit means or do not conform with or satisfy any or all of the
conditions of these terms and conditions; (iv) failure of any operations or
transmission of information, including without limitation, (a) any problems, (b)
human, production or technical errors, (c) lost, delayed, garbled or corrupted
data or transmissions, omissions, interruptions, deletions, defects or failures
of any telephone or transmission lines, (d) technical malfunctions of any
computer on-line systems, servers, access providers, computer equipment, or
software, or (e) failure of any e-mail to be received on account of technical
problems or traffic congestion on the Internet or at any website, or any
combination thereof (including without limitation, any injury or damage to any
person's computer related to or resulting from participating in or downloading
any material connected to this Offer, all of which may affect a person’s ability
to participate in the Offer); (v) any typographical or other error or omission
in any printing or advertising relating to this Offer, in the administration or
execution of this Offer; (vi) the security or privacy of information transmitted
via computer networks or for breaches of privacy due to interference by third
party computer "hackers" or otherwise; (vii) any claims regarding the accepting,
arranging for, awarding of or use of a Card; (viii) electronic communications
which are undeliverable as a result of any form of active or passive filtering
of any kind; or (ix) fraud by any participant. Sponsor reserves the right, at
its sole discretion, to amend these terms and conditions for clarification
purposes, at any time, in any way, and without prior notice, without materially
affecting the terms and conditions of this Offer. Without limiting the
foregoing, if for any reason the Offer is not capable of running as described
herein, including should any virus, worm, bug, technical failures, unauthorized
human intervention, or other causes corrupt or affect the administration,
security, or proper conduct of the Offer, Sponsor reserves the right to modify,
terminate or suspend this Offer (or portion thereof). In the event of
termination, Sponsor may honor non-suspect redemptions received up to the date
and time of such termination, or up to the date and time of the situation
warranting such termination. Sponsor has no obligation to provide, or
acknowledge requests for, a Card to anyone who has not strictly complied with
these Offer terms.
