Canada and managed by a third-party service provider, Specialty Health Network by Shoppers Drug Mart. A program representative will assist with filling out insurance forms, if required, and provide further information about the program. It will take you less than 5 minutes to complete your enrolment process. Please have the following information handy to complete the enrolment: - your health card, - your physician’s contact information, and - your private drug and/or provincial coverage information.
