Dear City of Mountain Brook Resident,
The City of Mountain Brook is requesting your help. You have been chosen to
participate in a survey designed to gather resident opinions and feedback
regarding City services and programs. Every three years, we plan to ask a
randomly selected group of Mountain Brook residents to complete a survey, which
is designed to gather residents' feedback about current city government
activities. The information requested in this survey will be used to help
improve our services and determine the future needs of Mountain Brook's
residents.

We greatly appreciate your participation. We estimate it should take
approximately 15-20 minutes to answer all the questions, and your individual
responses to the questions will remain confidential. The address on the survey
identifies responses from broad geographic areas and helps us know where we
might improve our service delivery. When we did our last survey, three years
ago, we received over 45% response rate. Our goal this year is to receive over
50% response rate.

The survey data will be compiled and analyzed by ETC Institute, one of the
nation's leading governmental research firms. ETC representatives will present
survey results to the Mountain Brook City Council later in 2020. If you have any
