WELCOME TO COMMUNITY HEALTH SERVICE INC!

Community Health Service Inc. is a private, nonprofit network of clinics
providing quality primary healthcare in Minnesota and North Dakota. As a
federally qualified health center (or FQHC), CHSI is able to provide a variety
of medical, dental, behavioral health, enrollment, and victim advocacy services
to our communities at low cost. We believe that everybody deserves quality
healthcare, so we work to provide it and reduce barriers — and we're proud to
measure our success in these efforts since we formed in rural Minnesota in 1973.

Today CHSI comprises four clinics located in Moorhead, Rochester, Willmar (MN),
and Grafton (ND). Our Victim Advocacy Program also maintains an office in
Crookston, MN.
