These Terms and Conditions of Use (the “Terms & Conditions”) govern your use of
this website and your administration of any MHS assessment instrument (the
“Instrument”). By clicking on “I Accept” below, you confirm that you have read
these Terms and Conditions and that you agree to be bound by them.

PERMITTED ACCESS

You have been provided a password to enter this website for the purpose of
administering the Instrument.

DATA COLLECTED

“Personal Information” is information about an identifiable individual.

As an administrator, we may collect a name, phone number, email address,
qualification level or certification information from the qualified customer for
the purposes of granting access to the site to administer, score and generate
reports for your clients.

The test user completing an assessment through the MHS online assessment site,
may need to provide contact information such as a name, company name and/or
email address. Alternatively, with the open link available within the MHS Online
Assessment platform, you may choose to provide your client with an invitation
through your office mail therefore not requiring this personal information.

Response Data: When a client is provided a link to complete an assessment, MHS
will save all response options selected. The client has the option to skip/not
answer some or all of the items, however it may not be able to score and
generate a report if too many item responses are omitted. Personal Data: MHS
processes assessment data (personal and response) for the purpose of providing
the requested services, such as processing the received data in order to
identify the assessments associated with an MHS account, as well as providing
corresponding reports.

As part of the assessment process, the client will be asked to provide
information used to score and generate a report. Examples of data that may be
asked: name, gender, age, ethnicity, country and occupational information. Some
data is mandatory in order for the results to be generated while other data is
optional and has been identified as such. Additional information may be
requested dependent on the assessment being administered.

MHS recommends using identification numbers instead of an individual’s first and
last name when completing the assessment. The administrator will be responsible
for assigning and tracking the use of any identification numbers.

The Administrator is entitled to use the personal information that we provide as
part of our services, however, is obliged to process such personal information
in accordance with your own obligations under data protection laws (as
applicable) and in accordance with these Terms and Conditions.

INFORMATION PRIVACY

Certain information will be provided to you by MHS for the purposes of scoring,
analyzing, and interpreting the Instrument’s results. You acknowledge that MHS
has no control or influence over your use of the Instrument’s results,
information (including any respondent personal information such as name, age,
responses to items, scores, and other similar data), or reports generated after
the Instrument is completed.

MHS is committed to protecting the privacy and security of the information
transmitted on this website and it employs technology that secures the
information and prevents unauthorized access, use, or disclosure. You
acknowledge that the internet is not an absolutely secure medium, and the
privacy of your information, communications, and visits to this website cannot
be guaranteed. Please refer to MHS privacy policy,

Data collected and received by MHS in connection with the delivery of
assessments will be transmitted and stored on private servers in the United
States with a third-party cloud-services provider (CSP) who complies with the
U.S.– E.U. Privacy Shield Framework. MHS’ employees and those of its service
providers who require such Personal Information for the purposes of their duties
may have access to this file. MHS and its service providers may be located
outside your jurisdiction of residence and such Personal Information may be
transferred to such jurisdiction and be subject to the laws of such
jurisdiction. Solely to the extent provided by law, you may request to have
access to the file containing any such Personal Information and the correction
of this Personal Information, by contacting MHS at privacyofficer@mhs.com.

The Internet is a global environment. By using our sites and sending information
to us electronically, you consent to trans-border and international transmission
of any personal information collected or processed through our sites.

As a qualified customer and test administrator you agree to obtain consent from
the respondent (participant), and if applicable, their parents or guardians, for

Conditions, which will remain valid and enforceable.
