GENERAL PSYCHOLOGY

Evaluation and treatment of Reactive-Situational Anxiety and/or Depression,
Generalized Anxiety, Panic Attacks, Persistent Depression, Major Depression,
Social Anxiety (with or without underlying Avoidant Personality), and
Post-Trauma Stress due to a single severe life stressor or traumatic loss

Consult-liaison with Psychiatry for the conditions above when medication is
indicated, concurrent with psychotherapy

Dr. Burkhart's primary psychotherapy approach is informed
by Process-Experiential/Emotion-Focused Therapy (PE-EFT) and he adheres to
its six treatment principles: achieving empathic attunement, fostering an
empathic and caring therapeutic bond, facilitating task collaboration, helping
clients process experience appropriately to the task, supporting completion of
key client tasks, and fostering  client development and empowerment. In the
context of PE-EFT when indicated, he utilizes treatment methods and protocols
from  Cognitive-Behavioral Therapy (including Trauma-Focused CBT / Trauma
Narratives), 
Acceptance and Commitment Therapy (ACT), and Functional Analytic
Psychotherapy (FAP). And he provides instruction on and practice with breath
control (relaxation / fight-or-flight reduction breathing) and mindfulness
meditation; also for Trauma Narrative Processing-Reprocessing assist, left-right
eye movement via Virtual Walks & Hikes


Please note that if you are experiencing or wanting help with a potentially life
threatening emergency (acutely dangerous suicidality or risk of harming others)
and not an established patient with Dr. Burkhart, you are advised to call 988

NEUROPSYCHOLOGY

Neuropsychological evaluation per Neurology or PCP  referral -standardized
neurocognitive testing (quantified measurement) of functions to include
attention, concentration, memory, language, spatial perception, and
neuro-executive skills (e.g. multitasking or divided and selective attention,
attentional and conceptual flexibility, planning, organization, step sequencing
and completion, and problem-solving); also neurobehavioral examination focusing
on decision-making, social judgment, impulse control, mood regulation, and
stress coping / stress tolerance)

Neuropsychological evaluation and medical consult-liaison to help referring
providers with differential diagnosis and prognosis pertaining to established or
suspected central nervous system conditions: pseudodementia (question of
depression vs. dementia), dementia such as Alzheimer's or vascular (ischemic
disease) or frontal lobe dementia (e.g. primary progressive aphasia), stroke
(CVA), Parkinson's, Multiple Sclerosis (MS), Hydrocephalus (NPH), and other CNS
degenerative diseases and transient organic brain syndromes

How to Make An Appointment -
If you are 18 years or older and interested in evaluation, consultation and/or
psychotherapy with Dr. Burkhart, please note that he is accepting new patients
but scheduling the first visit can be delayed up to 1-3 months .  You can call
him directly at 206-528-8534 and/or email him at DrBurkhart@waburkhartphd.com
and describe the specific symptoms or problems for which you are needing help,
along with any history or other background information which may be relevant to
your treatment needs and reasons for seeking help at this time.  Also helpful,
let him know what your funding source would be, what health insurance you have
or whether or not you would be paying privately. And if your treatment needs and
goals are a good match for his areas of specialty and expertise and if he takes
your insurance or private pay is not a problem, he will follow-up with you for
more information and discussion; and then if you and he agree to do so, an
appointment will be scheduled.
Patients or healthcare providers with referrals for a neuropsychological
evaluation, please start by calling Dr. Burkhart’s office at 206-365-1435 and
press 1 to leave a message for his patient intake coordinator (health insurance
authorization and scheduling). She will answer any questions you may have and if
you wish to proceed, schedule an initial appointment with Dr. Burkhart. She will
also explain to you what forms need to be completed and/or signed in preparation
for your visit (downloadable fillable forms posted below, which can be
downloaded to your computer, filled out on your computer, and then either
emailed to the office before your visit or printed and mailed or accompany you
to your visit).

CANCELLATION(S): Should you need to or decide to cancel your initial appointment
with Dr. Burkhart or any follow-up appointments you have scheduled, the courtesy
of a 72-hour notice is appreciated.  There will be no charge for appointments
canceled 48 hours or more in advance. Late cancellation and no-show fees cannot
be billed to your insurance company.  Please make every effort to keep your
appointment(s). While there are other patients scheduled and on a waiting list
to be moved up to fill cancelled appointments, 48 hours is usually required for
them to make all the necessary arrangements.
 
PAYMENT TERMS: If you have signed the New Patient Registration Form (Intake
Sheet) and authorized payment of health insurance benefits to Dr. Burkhart, it
is agreed that you will ultimately be responsible for any portion of amounts
charged, not paid by your insurance company.  If Dr. Burkhart is a participating
provider with your insurance, he has agreed to an allowed fee amount and after
your insurance is billed and paid their portion of that amount, you will be
billed for whatever co-insurance and/or co-payment is due from you. Your
insurance deductible may be applied and so you may end up being responsible for
the full amounts allowed by your insurance company. And should the insurance or
eligibility status listed on your Intake Sheet and initially verified by Dr.
Burkhart change by the time of your appointment and if by then you have not
provided and had verified alternative insurance, you will then be responsible
for any and all fee amounts charged.


GETTING TO BALLINGER PROFESSIONAL OFFICES:

From points south / traveling North on I-5: Take Exit 177 and follow signs
toward Lake Forest Park. Pass through the first intersection where you’ll see a
Chevron station on your right and head down Ballinger Way.

 From points north / traveling South on I-5: Take Exit 177 and follow signs
toward Lake Forest Park. At the end of the off ramp you’ll turn left and head
under the I-5 overpass and then pass through the first intersection where you’ll
see a Chevron station on your right and head down Ballinger Way.

Headed down Ballinger Way right east of I-5, you’ll drive past Ballinger Village
(and the Village Market Thriftway) on your left and then a second Chevron
station also on your left at the corner of the next intersection, which is 19th
Ave NE. Here, take a left and head up the hill to the middle of the block and
you’ll see Dr. Burkhart’s office on the right, a one-story olive-colored
building.

Parking is usually available in the lot in front of the building. Upon arrival
inside the office, please have a seat in the waiting room and you will be
greeted there, when it is time for your appointment.
