Report Sector IT Risk Incident



Organizations or individuals that are regulated by FSRA are expected to report a
“material” IT Risk incident.

For more information on what can be considered a “material” IT risk incident,
the timing to report an incident, as well as detailed information relevant to
your type of regulated entity/individual, please refer to the IT Risk Management
Guidance.



How to report a new incident



There are two options available to report a new incident to FSRA:

Option 1: Select the button to complete the online form.

Option 2: Upload a completed FSRA IT Risk Incident Notification Form or an
incident form from another regulator.

How to upload additional information



If you have already reported an incident to FSRA and a representative from FSRA
has requested that you submit supporting documentation, select the button to
upload a file.
