requesting the wire is not part owner of the account.


 * Substitute Form W-8BEN (PDF)
   
   A Form W-8BEN is to be completed by account holders that are non U.S.
   citizens and non-resident aliens. Failure to submit this form will result in
   maximum IRS-mandated tax withholding. Please note: A new Form W-8BEN must be
   completed every three calendar years and each time you change your address.
   If you provide a Form W-8BEN and you have a U.S. address/indicia, you will
   also need to complete the below Letter of Explanation (LOE)
   
   
   
   
 * W-8BEN Letter of Explanation (PDF)
   
   A Foreign Tax Identifying Number is required on all W-8BENs; if no Foreign
   Tax ID is available the attached Letter of Explanation should be completed.
   
   
   
   
   
 * Substitute Form W-9 (PDF)
   
   A form W-9 is generally completed by account holders that are U.S. citizens,
   including resident aliens. Failure to submit this form will result in maximum
   IRS - mandated tax withholding.  
   
   
   
   
   
 * Letter of Authorization to Transfer Assets by Wire (USD) to a U.S.
   Bank/Financial Institution (PDF, MS Word)
   
   This form authorizes Morgan Stanley to transfer U.S. dollars generated from
   the exercise and sell of your stock options in StockPlan Connect to another
   account or financial institution in the form of a wire transfer as specified
   by you. This is a standing instruction that does not expire. These
   instructions will remain in place until modified or restricted by you. The
   funds will not be wired automatically unless instructed at the time of the
   trade. This form is not valid for transfer of funds other than U.S. dollars
   resulting from proceeds converted by Worldlink.
   IMPORTANT: Please note that this form is ONLY to be used for transfer from
   StockPlan Connect account, transfers from a Morgan Stanley Access Direct

If you wish to transfer your securities, complete one of the following letters
of authorization to Transfer Assets (Securities).  By completing this form, you
are authorizing Morgan Stanley to transfer (DTC) assets from your stock plan
account to another Morgan Stanley account specified by you or to another
financial institution.

 *  Title Change Form (PDF, MS Word)
   Please Note: Once you finish updating your title on file, you will also need
   to re-certify your tax status by completing a new W-8BEN or W-9 that includes
   your new title.  Please see the Tax Certification section of this web page.  
   
 * Change of Address Form (PDF, MS Word)
   
   A paper form is ONLY required for joint accounts.  If you have a joint
   account, please complete the form and fax to the number listed on the form.
    Single account holders may contact our Service Center at the telephone
   number located on your statements.
    
   Please Note: Once you finish updating your address on file, you will also
   need to re-certify your tax status by completing a new W-8BEN or W-9 that
   includes your new address.  Please see the Tax Certification section of this
   web page.  
   
   
 * Letter of Authorization to Convert and Transfer Funds (PDF)
   
   This form is used to allow participants to receive proceeds in currencies
   other than US Dollars after the sale of stock. WorldLink provides exchange
   rates and distributes the proceeds to the participants in the form of a
   currency check or currency wire. Currency conversion wires are allowed for
   ONLY certain non-US corporate clients. To receive a currency conversion wire,
   Morgan Stanley must offer the desired local currency. Your company plan must
   offer currency conversion, and a Form W-8BEN must be on file and approved at
   time of trade. Please ensure the WorldLink Wire Form is completely filled
   out. Missing information will cancel out the currency conversion wire, and a
   US Dollar check will be mailed to the address of record instead. You should
   check with a Morgan Stanley Service Professional to verify if your company
   offers this method and the desired currency before submitting this form.
    
   
 * Request for Certificate (PDF, MS Word)
   
   The Direct Registration System (DRS) is a free alternative to stock
   certificates that registers your shares directly on the books of the company
   transfer agent without the need for a physical certificate to prove
   ownership. You do not receive a physical certificate, but instead receive a
   statement of ownership and periodic account statements (at least yearly).
   Dividend or interest payments, proxy materials, annual reports, and other
   pertinent materials are mailed to you from the issuer or transfer agent.  
   Please ensure the DRS Transfer Form is completely filled out. Missing
   information will delay processing the DRS transfer. You should check with a
   Morgan Stanley Service Professional to verify if your company offers this
   method and the desired currency before submitting this form.
   
   NOTE: Transfers through the Automated Customer Account Transfer Service
   (ACATs) are not available.

 * How do I access my account online?
   
   If you haven’t already, you can register your account online at
   stockplanconnect.com using the temporary user name and password sent to you
   by mail. If you do not have this information available, please click on the
   ‘Register Your Account’ link in the login box. You will be asked a series of
   qualifying questions to validate your identity. Once your identity has been
   confirmed, link to the registration process will be emailed to you. This link
   will be valid for 30 minutes.
   
   
   
 * What do I do if I can’t remember my password?
   
   If you forget your password, go to www.stockplanconnect.com and select the
   “Forgot Password” link in the login box. You will be asked a series of
   qualifying questions to validate your identity. Once your identity has been
   confirmed, a link to reset your password will be emailed to you. This link
   will be valid for 30 minutes.
   
   
   
 * What do I do if I can’t remember my user name?
   
   If you forget your user name, go to www.stockplanconnect.com and select the
   “Forgot User Name” link in the login box. You will be asked a series of
   qualifying questions to validate your identity. Once your identity has been
   confirmed, a link to view your user names will be emailed to you. This link

 * What do I do if I can’t remember my Voice PIN?
   
   If you forget your Voice PIN, go to www.stockplanconnect.com and select the
   “Forgot Voice PIN” link in the login box. You will be asked a series of
   qualifying questions to validate your identity. Once your identity has been
   confirmed, a link to reset your Voice PIN will be emailed to you. This link
   will be valid for 30 minutes.
   
   
   
 * Where can I find my Global Identification number (Global ID/GID)?
   
   A Global ID/GID is a nine digit numeric identifier assigned by your company.
   Please contact your company to obtain this number.
   
   
   
 * I’m having problems accessing my account online, what should I do next?
   
   The StockPlanConnect website requires certain web browsers, settings and
   software. Please see the following list of recommended browsers, and browser
   versions:
   
   * Chrome 71 & above
   * Safari 12 & above
   * Firefox 64 & above
   * Microsoft Edge 40 & above
   * Internet Explorer 11 (not optimal)
   
   
 * Is my information secure?
