on the use of invasive BiPAP—the use of a hospital grade BiPAP device for

WHY IS INVASIVE BIPAP VENTILATION AN IMPORTANT OPTION FOR COVID-19 PATIENTS
SUFFERING RESPIRATORY INSUFFICIENCY?

 
 


A critical shortage of mechanical ventilators is anticipated in many countries,
including the United States, because of the ongoing COVID-19 pandemic. Evidence
suggests that bi-level positive airway pressure (BiPAP) support may be used in
the hospital setting with appropriate precautions for intubated COVID-19
patients experiencing Acute Respiratory Distress Syndrome (ARDS).1,2

On March 22, 2020 the U.S. Food and Drug Administration released guidance
allowing the use of certain devices, including noncontinuous ventilators
delivering BiPAP, to treat patients with respiratory insufficiency on an
emergency basis provided that appropriate design mitigations are in place to
minimize aerosolization.

Use of invasive BiPAP on an emergency basis may delay or remove the need for
mechanical ventilation for COVID-19 patients in the hospital setting with
appropriate precautions.

NIH Guidance

NATIONAL INSTITUTES OF HEALTH TREATMENT GUIDELINES: CARE OF CRITICALLY ILL
PATIENTS WITH COVID-19

The National Institutes of Health (NIH) provides summary recommendations for
treatment of critically ill patients with COVID-19 covering infection control,
hemodynamic support, ventilatory support and drug therapy.

HHS Guidance

OPEN LETTER FROM THE ASSISTANT SECRETARY FOR HEALTH AND THE U.S. SURGEON GENERAL
ON OPTIMIZING VENTILATOR USE DURING THE COVID-19 PANDEMIC

The Department of Health and Human Services (HHS) Assistant Secretary for Health
and U.S. Surgeon General issued an open letter to the healthcare community
stressing the need to aggressively implement measures to optimize ventilator use
during the COVID-19 pandemic, stating that “BiPAP may be used for invasive
ventilation.”

 AARC Guidance

GUIDANCE DOCUMENT FOR SARS COV-2

The American Association for Respiratory Care (AARC) has issued guidance to the
medical community to address ventilation options for patients with COVID-19,
including specific recommendations for use of invasive BiPAP.

Recommendations relevant to invasive BiPAP

 * “Bilevel devices are ventilators and can be used for invasive ventilation.”

 * “In severe respiratory distress, do NOT delay intubation.”

Recommendations against certain other modalities

 * “Do not attempt to ventilate multiple patients with a single ventilator.”

 * “Because of high failure rate and the possibility of environmental
   contamination, we suggest avoiding NIV” (non-invasive ventilation).
