REQUIRED COVID-19 RAPID TESTING PROGRAM

With your return to in-person teaching and learning, research, or working on
campus, or if you have been working on campus throughout the pandemic, UBC
remains committed to protecting your safety and wellbeing and that of all
members of our community.

UBC requires COVID-19 rapid testing for all students, faculty and staff, with
exemptions provided for those who are fully vaccinated against COVID-19.

As the first step of the COVID-19 Rapid Testing Program, all students, faculty
and staff are required to complete an online, confidential declaration of their
vaccination status. This information will be used to determine whether you are
required to participate in UBC’s COVID-19 rapid testing program or are eligible
for an exemption.

Those who indicate that they are unvaccinated, partially vaccinated, or do not
disclose their vaccination status, are required to take part in regular COVID-19
rapid testing. To complete your Vaccination Status Declaration, you need your
Campus Wide Login (CWL) and complete the following steps.

 1. You will be prompted to answer a question about your vaccination status.
 2. If you declare that you have been fully vaccinated, you are required to
provide verification of vaccination and will not need to take part in
COVID-19 rapid testing. Audits will be conducted as part of the program, and
students, faculty and staff may need to provide additional information upon
    request.
 3. If you indicate that you are not fully vaccinated, or you choose not to
disclose your status, you will need to take part in COVID-19 rapid testing
when working or studying on one of UBC’s campuses.

PROOF OF VACCINATION

If you have declared that you are fully vaccinated, you will be asked to upload
your BC Vaccine Card to the declaration website to verify that you have been
vaccinated. Instructions on getting your BC Vaccine Card are available here.
Your card should look like this:

If you are not currently eligible to get a BC Vaccine Card, or are awaiting
access, then you may upload other documentation that shows your full name, name
of vaccine(s) and date(s) administered. You may upload multiple documents if
necessary. If you were vaccinated in a country other than Canada, you should
upload the proof of vaccination which was required in order for you to enter
Canada; if your document is not in English or French, then you must upload a
copy of both the original language proof of vaccination documentation and an
officially translated copy.

Your proof of vaccination must be uploaded in .PDF, .JPG, or .PNG format.

If you cannot access the declaration website or have questions about what is
acceptable, contact ready.ubc@ubc.ca to determine an alternative means of
submitting your proof of vaccination.

COVID-19 RAPID TESTING

If you are a student, faculty member, or staff member and are not fully
vaccinated, or prefer not to disclose your status, you will need to take part in
COVID-19 rapid testing when working or studying on one of UBC’s campuses. If you
are working or studying at home or at an off-campus facility controlled by UBC,
you are not currently required to take part in COVID-19 rapid testing.

Rapid Testing Requirements:

 * Rapid testing is only available to those who are symptom-free. For
information about what to do if you have any symptoms suggestive of COVID-19,
please visit: https://www.healthlinkbc.ca/covid-19/testing and follow the
   guidance related to testing.
 * Rapid testing is not available to those who tested positive for COVID-19
   within the past 90 days.
 * The rapid test is performed via a shallow nasal swab.
 * You are required to attend one rapid testing appointment every week.
 * Use the link below to make your appointments. Appointments are required. No
   walk-ins are permitted.
 * Bring photo ID and your Provincial Health Number to your appointment.
 * There is one clinic on each campus. Locations and hours of operation are
provided in the online appointment website.
 * The UBC COVID-19 Rapid Testing site at the Irving K. Barber Learning Centre,
on the Vancouver campus, is a separate, voluntary, research-based initiative.
It is not part of the required UBC COVID-19 Rapid Testing Program and you
should not attend at this site unless you are taking part in that research
   initiative.

COVID-19 rapid testing is not considered a substitute for vaccination. We
strongly encourage all eligible students, faculty, and staff to get vaccinated.
For more information about BC’s immunization program and how to obtain a
vaccination please
visit: https://www2.gov.bc.ca/gov/content/covid-19/vaccine/register.
