Survey Overview

This survey is designed to help understand how to make Smart a healthy, high
performing organization. Smart needs your opinions and assessments to identify
strengths to build on, opportunities for improvement, and obstacles standing in
the way of greater success.

This OHI survey is being conducted on behalf of Smart. If the survey collects
sensitive personal data, your sensitive personal data will be collected and
processed on the basis of your consent which will be requested immediately
before such questions. McKinsey & Company, on behalf of Smart, will analyze your
responses directly and by comparison to McKinsey’s benchmarking database. Your
responses are treated as confidential, and the results of the survey and
