Furey , Joseph DiDomenico , Joseph Abbatematteo , Donald Detchou , Joshua S. Catapano , Laura A. Snyder , Michael T. Lawton , Ann Stroink , and Paul M. Arnold IN BRIEF This study investigated reimbursement patterns to neurosurgeons in the United States by private companies, and the geospatial distribution patterns nationally to identify regions with greater and reduced reimbursement, demonstrating the presence of disparities, influenced by socioeconomic and demographic parameters. Publication Date: 27 Oct 2023 DOI link: https://doi.org/10.3171/2023.8.JNS23276 Restricted access Use of global sagittal flexibility to predict proximal junctional kyphosis after spinal deformity surgery IN JOURNAL OF NEUROSURGERY: SPINE Ryan G. Eaton , Joshua L. Wang , Vikas Munjal , Joravar Dhaliwal , Dominic Maggio , Alexander Keister , David Gibbs , Noah Mallory , Alexander Sparks , Robert Moranville , David S. Xu , Andrew J. Grossbach , and Stephanus Viljoen IN BRIEF Researchers analyzed the effect of sagittal flexibility on the postoperative outcomes of 85 patients with adult spinal deformity who underwent long-segment fusion with pelvic instrumentation. Rigid patients, defined as those with less than a 3.7-cm change in sagittal vertical axis from supine to standing, were at increased risk of radiographic proximal junctional kyphosis at 1-year follow-up. This study highlights the importance of understanding a patient's standing and supine alignment when planning deformity correction. Publication Date: 27 Oct 2023 DOI link: https://doi.org/10.3171/2023.8.SPINE23314 Restricted access Robot-assisted implantation of a microelectrode array in the occipital lobe as a visual prosthesis: technical note IN JOURNAL OF NEUROSURGERY Alessandra Rocca , Christian Lehner , Emmanuel Wafula-Wekesa , Enrique Luna , Víctor Fernández-Cornejo , Javier Abarca-Olivas , Cristina Soto-Sánchez , Eduardo Fernández-Jover , and Pablo González-López IN BRIEF Researchers used a novel technique to implant cortical microelectrodes through a robot-assisted procedure. Their preliminary results after stimulating the occipital cortex in blind volunteers are provided. This study showed the feasibility of implanting the Utah electrode array by robotic assistance, as well as the capability of this technology to evoke basic and complex visual perceptions. This is a novel technique that could lead to more precise implants and establish new indications. Publication Date: 27 Oct 2023 DOI link: https://doi.org/10.3171/2023.8.JNS23772 Restricted access Antithrombotic therapy and spinal surgery: a retrospective cohort study of 289 consecutive elderly patients with degenerative lumbar stenosis IN JOURNAL OF NEUROSURGERY: SPINE Giuseppe Corazzelli , Mara Capece , Valentina Pizzuti , Settimio Leonetti , Alessandro D’Elia , Marco Santilli , Fulvio Aloj , and Gualtiero Innocenzi IN BRIEF This study aimed to research differences in blood loss, operative time, and postoperative hospitalization in patients discontinuing or not discontinuing antithrombotic therapy before elective lumbar decompression. This study found no statistical differences in these variables between patients discontinuing and those not discontinuing their antithrombotic therapy prior to decompression for lumbar spinal stenosis. Additionally, no early complications were recorded. These findings suggest that antithrombotic therapy may not be discontinued during elective decompression for lumbar spinal stenosis. Publication Date: 27 Oct 2023 DOI link: https://doi.org/10.3171/2023.8.SPINE221332 Restricted access Concurrent impact of de novo mutations on cranial and cortical development in nonsyndromic craniosynostosis IN JOURNAL OF NEUROSURGERY: PEDIATRICS Emre Kiziltug , Phan Q. Duy , Garrett Allington , Andrew T. Timberlake , Riki Kawaguchi , Aaron S. Long , Mariana N. Almeida , Michael L. DiLuna , Seth L. Alper , Michael Alperovich , Daniel H. Geschwind , and Kristopher T. Kahle IN BRIEF The authors performed a functional genomics analysis of nonsyndromic craniosynostosis (nsCS) to determine when, where, and in what cell types nsCS-associated genes converge during development. They found that nsCS genetic risk converges in cells and transcriptional networks that concurrently impact skull, meninges, and brain development. These findings suggest a primary impact on neurodevelopment in nsCS, providing an explanation for why some children with nsCS continue to have neurodevelopmental disability despite correction of cranial defects. The authors hypothesize that genetic factors may be more important for determining overall neurodevelopmental outcome than either the timing or type of surgical correction used (endoscopic vs open). Publication Date: 27 Oct 2023 DOI link: https://doi.org/10.3171/2023.8.PEDS23155 Restricted access Effect of thin-split encephalomyosynangiosis on transient neurological events in revascularization surgery for pediatric patients with moyamoya disease IN JOURNAL OF NEUROSURGERY: PEDIATRICS Kazuki Ishii , Fumiaki Kanamori , Yoshio Araki , Kenji Uda , Kinya Yokoyama , Takashi Mamiya , Kai Takayanagi , Shunsaku Goto , Masahiro Nishihori , Takashi Izumi , and Ryuta Saito IN BRIEF This study aimed to evaluate the effects of thin-split encephalomyosynangiosis (EMS) in combined revascularization surgery on postoperative outcomes in pediatric patients with moyamoya disease (MMD). The authors found that thin-split EMS can reduce transient neurological events that do not accompany crying or hemodynamic changes. Furthermore, the procedure has sufficient prevention effects against stroke events during the chronic postoperative period. The thin-split EMS method could be valuable for revascularization in pediatric patients with MMD.
