DSC FINANCIAL ASSISTANCE POLICY

DSC Financial Assistance Policy Every patient must pay their part of estimated
financial liability on or before the day of service. We may request that your
deductible and copay amounts are paid on or before your date of surgery. If the
patient is unable to pay the liability portion in full, the Doctors Surgery
Center (DSC) may offer a short term repayment schedule after a minimum down
