<p><strong>Assessing Research Climates Consent Agreement</strong></p>
<p><strong>What is the purpose of this study?</strong></p>
<p>You are invited to participate in a research study of the interpersonal and research climates at your institution, managed by the National Center for Principled Leadership and Research Ethics (NCPRE) at the University of Illinois
Urbana-Champaign. The purpose of the research is to establish norms for a series of validated measures of the institutional climate across institutions and fields of study, and to provide institutional leaders with aggregated information about
how the members of the organization perceive its climate. Your participation in this survey will last about 15-25 minutes and involves answering a series of survey questions about your work environment. Risks related to this research do not
include anything beyond those experienced in everyday life; the survey includes questions about the interpersonal climate of your workplace, which can be a sensitive topic. It does not include any questions about specific
experiences. Benefits related to this research include contributing to efforts to improve our understanding of interpersonal and research climates in research-intensive organizations to improve the science done in those organizations.</p>

<p><strong>Why am I being asked to participate?</strong></p>
<p>Each person has their own perspective about the climate in a place of work. Leaders at an institution will have their own perspectives. By evaluating anonymized results in aggregate, your institution's leadership can base its
decisions on evidence, rather than any one person's individual perspective, which is likely to lead to better outcomes. The more people that respond, the better the quality of the information. The maintenance and improvement of
positive climates for work is thus directly related to each individual's participation.</p>
<p>NCPRE's surveys are run at institutions across the US and in a handful of locations internationally. The various instruments have been fielded to many: for instance, tens of thousands of participants have taken the SOURCE, and
thousands have taken the CARES Survey.</p>
<p><strong>Can I refuse?</strong></p>
<p>Your participation in this research is voluntary. Your decision whether to participate will not affect your current or future dealings with the University of Illinois Urbana-Champaign, nor with your institution. If you decide to participate,
you are also free to withdraw at any time without affecting those relationships.</p>
<p><strong>What procedures are involved?</strong></p>
<p>The study consists of a single online survey, largely multiple choice, that will take 15-25 minutes to complete. Depending on the question sets in use, they may address aspects of the research climate, the interpersonal
climate, satisfaction with your circumstances, or perceptions of behavior.</p>
<p>Participants answer the questions on NCPRE's Results Analysis Engine, a web-based app developed at the University of Illinois. The Engine will report aggregated results of the survey to your institution, with individual responses totaled in
groups at department or similar organizational levels. No individual data will be reported and no data from groups with fewer than five people responding will be reported. The research team may publish aggregated results in reports or journal
articles. Project personnel at the University of Illinois who may see your information will maintain confidentiality to the full extent permitted by laws and university policies. Apart from initial questions that select which survey
sections apply to you, you may decline to answer any question you don’t wish to answer. Once you press the “Finish” button at the end of the survey, the Engine separates your answers from any of your identifying information and your identifying
information is deleted. If you do not finish the response, your identifying information is removed once the survey fielding window is closed.</p>
<p><strong>Will my study-related information be kept confidential?</strong></p>
<p>We make reasonable efforts to keep your personal information confidential, but we cannot guarantee absolute confidentiality. When this research is discussed or published, no one will know that you were in the study. When required by law or
university policy, individual responses to the survey may be seen or copied by: a) The Institutional Review Board that approves research studies; b) The Office for Protection of Research Subjects and other university departments that oversee
human subjects research; c) University and state auditors responsible for oversight of research. Individual responses to the survey may also be seen by members of the NCPRE research team during programming, analysis and report generation
activities; our separation of individual-level identifying data from your survey responses as early in the survey process as possible is designed to protect the confidentiality of your responses, even with respect to the research team.</p>
<p><strong>Will I be reimbursed for any expenses or paid for my participation in this research?</strong></p>
<p>You will not be offered payment for being in this study.</p>
